Rapid Training Waiver
I hereby, authorize any and all medical attention deemed necessary in the event of an accident, injury, sickness, etc. under the direction of the bearer of this form. This release is effective until revoked by me and I thereby assume responsibility for payment of such treatment. I hereby give my consent to participate in Rapid Sports Performance Training. I understand that injuries can occur during participations in athletic training. I/We herby release and agree to hold harmless Rapid Sports Performance, LLC, and all coaches, officials, facility partners or employees for claims, for damages, for personal injuries or otherwise which may arise by virtue of acts, or omissions to act, negligent or otherwise, on the part of Rapid Sports Performance LLC. It is our specific intent to fully indemnify and hold harmless Rapid Sports Performance LLC, and its coaches, officials, facility partners and employees, without limitation of any kind as to party, nature of claims, amount of claims, or time interval involved.
This is to certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above of all the Releasees, and for myself, my heirs, assigns, and next of kin, I release and agree to indemnify the Releasees from any and all liabilities incident to my minor child’s involvement or participation in these programs as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE.
I give Rapid Sports Performance permission to publish in print, electronic, or video format the likeness or image of myself and/or my child, for the general promotion of Rapid Sports Performance and its programs.
CANCELLATIONS, NO-SHOWS, SESSIONS EXPIRATION POLICY
We understand that homework, sickness, and other unexpected things happen from time to time. If cancelling a session, it is our expectation that the parent or client contact us within 12 hours of our scheduled training session. Not contacting your trainer or not showing up without contacting them will result in us counting that day as a session trained. When cancelling, please provide us with other available days/times during that same week the athlete can come in so we can reschedule the cancelled session. Note: This only applies to small group training; this does not apply to any team training. In order to ensure optimal results, training frequency is crucial. Our expectation is that 8 sessions must be completed within 4 weeks or unused sessions will expire. Again, in the event that a scheduled session is cancelled, we encourage parents/players to reschedule their training session for another day & time during that same week. With other small training groups going on a daily basis, it is very likely that the player can jump into one of these sessions to keep their sessions on track and continue with their ongoing progress. Please understand we would like to keep these situations to a minimum so that we don’t have too many players jumping around from session to session.
